Premiums for employer-sponsored family health insurance increased an average of 41 percent across states from 2003 to 2009, more than three times faster than median incomes, and Maryland’s rise was among the highest, at 50 percent, according to a report to be released Thursday by the Commonwealth Fund.

The report, which presents a state-by-state analysis of private employer health insurance costs for those six years, found that by 2009, the 10 jurisdictions with the highest annual total premiums included the District and Maryland. The average employer-sponsored family premium for all states was $13,027. In the District, it was $14,222; in Maryland, $13,833. Virginia ranked 32nd, below the national average, at $12,622.

Premiums for employer-sponsored coverage include the amounts paid by employers and employees combined.

The other states with high total premiums were Alaska, Connecticut, Massachusetts, Vermont, Wisconsin and Wyoming.

The Commonwealth report did not break down how the costs were split between employer and employee over the years.

The Commonwealth Fund supports the health-care overhaul legislation. Its report Thursday said the new health-care law provisions have the potential to slow the rate of cost growth by giving states the ability to challenge excessive premium increases and by providing assistance for low- and middle-income families to pay for health insurance.

Industry analysts say, however, that figuring out how the new law will affect health-care costs, and therefore premiums, is among the trickiest issues surrounding the statute.

One of the lead authors of the report, Cathy Schoen, said health insurance costs will tend to vary by costs of care and prices charged by doctors, hospitals and diagnostic laboratories. Health-care costs tend to be somewhat higher in higher-income areas than lower-income states, she said.

In the District, Insurance Commissioner Gennet Purcell said the higher health insurance costs reflected the higher costs of doing business in the city. “The salaries here are higher, the cost of doing business is higher, and you can’t pay [in premiums] what you pay in Iowa,” she said. In addition, the District is an urban area and hard to compare to a state where the costs of living might be lower in some rural areas.

Beth Sammis, acting commissioner for the Maryland Insurance Administration, said she did not know what was behind the increase in private-sector health insurance costs. Her office reviews requests for rate increases from insurance companies. But many of the state’s large employers, such as Lockheed Martin, Northrop Grumman and Marriott, are self-insured and therefore not regulated by the state.

The report also found that deductibles rose sharply in almost all states, increasing an average of 77 percent from 2003 to 2009 in large as well as small firms. In addition, more workers are paying deductibles; 74 percent faced a deductible in 2009 compared with 52 percent in 2003.

Schoen said the rapid increase in health insurance premiums means that many working families have been forced to trade off pay raises just to hold onto their health benefits. The expanding share of premiums paid by workers themselves has also taken a greater cut out of paychecks, she said.

Other recent studies have shown that employers are shifting health-care costs to workers to help ride out the economic downturn.

A survey released in September by the Kaiser Family Foundation and the Health Research and Educational Trust found that workers with health benefits are paying an average of 30 percent of the premium for family coverage and 19 percent of the premium for single coverage this year, the highest in 12 years of surveys by the two organizations.

Last year, workers were paying an average of 27 percent of the premium for family coverage and 17 percent for single coverage.

Controlling obesity with pharmaceutical drugs is one of the main approaches of the conventional (allopathic) medicine. On December 7, 2010, the advisory committee of the U.S. Food and Drug Administration (FDA) approved with 13- 7 votes a new weight lose pill. The drug is developed by the California based biopharmaceutical company Orexigen Therapeutics, Inc. and holds the name Contrave. It represents a combination of two already approved drugs- the antidepressant Wellbutrin (Bupropion) and Naltrexone, marketed under the trade names Revia and Depade, and used to fight addictions. “Contrave is designed to activate a hypothalamic center in the brain associated with reduced appetite, while blocking beta-endorphin, which may be responsible for limiting weight loss. We are also studying the effects of Contrave on related central pathways associated with the rewarding nature of select high calorie foods, ” stated the President and CEO of Orexigen

The expert panel concluded that Contrave is modestly effective and it can cause slight elevation in blood pressure and accelerated pulse. It may also cause dry mouth, constipation, headache, and mild insomnia. “We are encouraged by the panel’s view of the risk-benefit profile of Contrave. We look forward to continued discussions with the FDA regarding the design of a post-marketing study to evaluate cardiovascular risk and to address the questions rised at the meeting today in preparation for our PDUFA action date in January,” said Michael Narachi, president and CEO of Orexigen.

By following its final non-binding recommendation FDA is advising for another clinical trial in order to assess for long-term heart risks and final decision is expected in January, 2011. The expert FDA panel decision is based on the conclusion that the potential benefits of the new drug outweigh the potential risks when used long-term in a population of overweight and obese individuals.

Earlier this year three different companies (Orexigen, Arena and Vivus) competed for the FDA advisory panel final approval. Recently, the FDA sent a request to Abbott Laboratories in order the drug Meridia to be pulled from the market due to heart problems and temporarily denied its final approval of Qnexa and Lorcaserin over risks of psychiatric, cognitive or cardiovascular issues. The only available weight loss drug sold on the US market is Orlistat (Xenical) – a lipase inhibitor, also approved by the FDA.

On Wednesday, state Sen. Rodney Ellis (D-Houston) filed a bill that would require Texas health care providers to inform patients that blood drawn as part of any routine medical testing would be screened for HIV unless the patient opts out. Filed to coincide with World AIDS Day, the bill is based on CDC recommendations in 2006 that HIV testing be a routine part of health care for people ages 13 to 64.”I’m trying to take the stigma out of HIV/AIDS testing. If we can make HIV testing as commonplace as getting a physical or flu shot, I think we can reduce the toll of this disease in Texas,” said Ellis.
Ellis made his comments at a Houston AIDS meeting. There, officials told attendees that one-third of Texans diagnosed with HIV were tested late in the course of infection and that someone in Houston is diagnosed with HIV every seven hours.
Ellis sponsored a similar bill in 2009 but said he is optimistic that this session’s measure would do better since the issue has become more visible. Legislation sponsored by the state senator in previous sessions led to mandatory HIV testing for people being released from and entering Texas prisons. Last session, he sponsored a successful measure requiring HIV testing of pregnant women.
The new bill would require Medicaid and private insurance plans to cover the cost of HIV testing regardless of the reason the patient visits the physician.
Officials at the Texas Hospital Association and the Texas Medical Association expressed support for routine HIV testing. A TMA spokesperson, however, said the doctors’ group had concerns about the “mandate element” of last session’s bill, asking which physicians would be required to screen for HIV, and how often, and who would assure treatment for those found to be HIV-positive.
